Thank you for choosing and showing interest in giving birth at Hadassah Ein Kerem.
If you or your partner are not Israeli citizens, we kindly ask you to complete the following form before your delivery in order to register for childbirth. We recommend completing the form approximately two months prior to your expected due date.
After the birth, the father’s details will be completed based on the mother’s declaration and signature.
